Individual Professional Development Plan
The IPDP is your proposed program of professional development activities to complete for licensure renewal or converting a certificate to license as required by the Ohio Department of Certification and Licensure. The IPDP must be approved by the LPDC prior to the commencement of professional development activities used for licensure. Current Ohio law requires the completion of six (6) semester hours, the equivalent in CEU's (18) or a combination of both.
Who Must Complete an IPDP?
Anyone not holding a Permanent Certificate or anyone who has a Two-Year Provisional License.
How to Fill Out Your IPDP
Side 1
- Complete the Personal Information as requested.
- Complete the section related to your certificate(s) / license
- Certificate or License held refers to the grade level and/or subject endorsements: i.e. - Elementary 1-8, 7-12 Comprehensive Science
- Grade refers to the type of License: Provisional or Professional
- Issue Date refers to the actual date of issue by the ODE and is located in the upper right hand corner of your certificate / license and becomes an important date as no professional development activities for your next license can begin before this date.
- The Expiration Date, by law, will always be June 30 of the year your license/certificate expires.
- Check the appropriate column indicating your plan to renew existing certificate/license or to convert to a 5-Year License.

- Complete Name and Date as requested
- Plan duration should cover the length of your current certificate / license. If you have multiple certificates / licenses, the date should cover all credentials as only one IPDP is required per renewal cycle.
- Complete your Personal / Professional goals for the plan. (Goals should be clearly stated and focused, i.e.: to study cooperative learning strategies, to acquire knowledge about thematic teaching, etc.)
- Complete district / building goals addressed by your Personal / Professional goals as there should be some connection between the two. (Check the Strategic Plan, Continuous Improvement Plan, and building level administrators for appropriate goals)

- List your proposed professional development activities to complete the requirements of your plan. Your activities must include all possible professional development activities including coursework, workshops, and approved local professional development activities as listed on the CEU Activities Guideline. You may revise your IPDP if your plans need to change.
- Describe how your plan and professional development activities will impact you, student achievement, and/or the quality of the district.

- Complete an IPDP at the beginning of your license cycle.
- Keep documentation of professional development activities in portfolio.
- Make copies of all LPDC forms, IPDP and professional development activities for your portfolio.
