Student Registration


Your child’s first day of school will be two (2) business days after you have completed the enrollment process at their school. You will be responsible for transporting your child(ren) to/from school until bus service has been arranged. The transportation department will contact you with the bus number, bus stop location, and the bus pick-up and drop-off times as soon as that has been established. Please allow 3 business days for transportation to be implemented.


Students who are enrolling in Grades 6-12 will have an appointment set for them to meet with a school counselor to place them in the appropriate classes once they have finished the enrollment process at their school office.

 

Parents will need to bring with them the completed registration packet and the following documentation. Please note that registrations will not be accepted without all required documents.

  • CHILD’S ORIGINAL BIRTH CERTIFICATE (photocopies are not acceptable)
  • CHILD’S SOCIAL SECURITY CARD
  • CHILD’S IMMUNIZATION RECORD
    • DPT (diphtheria, pertussis, tetanus) – Kindergarten: 5 doses of DTaP or DT or any combination, if the 4th dose was administered prior to the 4th birthday.  Grades 1-12: 3-4 doses of DTaP, DTP, DT, or Td or any combination.Grade 7: 1 dose of Tdap or Td vaccine must be administered prior to entry.
    • PolioKindergarten: 4 doses of any combination of POV or IPV, the final dose must be administered on or after the 4th birthday regardless of the number of previous doses.  Grades 1-12:  4 doses if a combination of OPV or IPV was administered.  4 doses of all OPV or all IPV is required if the 3rd dose of either vaccine was administered prior to the 4th birthday.
    • MMR (measles, mumps, rubella) - Grades K-12: 2 doses.  The 1st dose must be after age 1 year and the 2nd dose must be given at least 28 days after the 1st dose. 
    • Varicella (chicken pox) – Kindergarten: 2 doses must be administered prior to entry to school.  Grades 1-4: 1 dose of varicella must be administered on or after the 1st birthday.
    • HepB (hepatitis B) – Grades K-11: 3 doses of Hepatitis B.  The 1st dose can be given any time after birth.  The 2nd dose must be 28 days after the 1st dose.  The 3rd dose must be given at least 16 weeks after the 1st dose and at least 8 weeks after the 2nd dose.  The last dose in the series must not be administered before age 24 weeks.

  • PROOF OF RESIDENCY
    We will accept the mailing portion of your water, phone, electric or cable bill, signed lease agreement or signed closing papers on your house (if signed within the last 30 days).  Proof must show your name and address. (Driver’s licenses, checks, bank statements and insurance documents are not acceptable.)

    Note: If you are building or buying a new home in the district and have not moved in yet, you must sign an affidavit stating the location and date you will be moved into your new home. For more information on the affidavit process see Residency Requirements.

  • SCHOOL TRANSCRIPTS (Grades 6-12 only)
