Springboro Community Schools’ Student Registration Office is now accepting Kindergarten registrations for the 2010-2011 school year.

 

The registration office is open from 8:00am – 12:30pm Monday-Friday. The office is located at Springboro Junior High, 1605 S. Main Street at Door 16 on the back side of the building.

 

A child is eligible for entrance into Kindergarten if s/he attains the age of five (5) on or before September 30 of the year in which s/he applies for entrance.

 

Mandatory KRA-L testing will be held August 16-18, 2010 at Clearcreek Elementary.  You will be notified by the building of your scheduled time over the summer months.

 

Kindergarten AM/PM assignments will be decided through the Transportation Department.  These assignments will be mailed to you by August 1, 2010.

 

Parents will need to bring with them the completed registration packet and the following information for registration.  Registrations will not be accepted without all required documents.

  • CHILD’S ORIGINAL BIRTH CERTIFICATE (photocopies are not acceptable)
  • CHILD’S SOCIAL SECURITY CARD
  • CHILD’S IMMUNIZATION RECORD
    • DPT (diphtheria, pertussis, tetanus) 5 doses of DTaP or DT or any combination, if the 4th dose was administered prior to the 4th birthday. 
    • Polio 4 doses of any combination of OPV or IPV, the final dose must be administered on or after the 4th birthday regardless of the number of previous doses. 
    • MMR (measles, mumps, rubella) 2 doses.  The 1st dose must be after age 1 year and the 2nd dose must be given at least 28 days after the 1st dose. 
    • Varicella (chicken pox) 2 doses must be administered prior to entry to school. 
    • HepB (hepatitis B) 3 doses.  The 1st dose can be given any time after birth.  The 2nd dose must be 28 days after the 1st dose.  The 3rd dose must be given at least 16 weeks after the 1st dose and at least 8 weeks after the 2nd dose.  The last dose in the series must not be administered before age 24 weeks.

     

  • PHYSICAL FORM

All kindergarten students must have a physical.  The physical form must be signed by the child’s physician.  This form should be received by the first two (2) weeks of school.

  • PROOF OF RESIDENCY

We will accept the mailing portion of your water, phone, electric or cable bill, signed lease agreement or signed closing papers on your house.  Proof must show name and address.  (Driver’s licenses, checks, bank statements and insurance documents are NOT acceptable.)

  • GUARDIANSHIP OR CUSTODY PAPERS (If Applicable)
