Request to Change Model Form - Quarter 2
On Monday, October 12, all families will receive an email with a link to a "Request to Change Model" Form for Quarter 2. This form should be filled out ONLY by those families wishing to request a change in models for Quarter 2 (Model A to Model B - or - Model B to Model A).
YOU DO NOT NEED TO FILL OUT THIS FORM IF YOU ARE STAYING WITH YOUR CURRENT MODEL. If you DO NOT wish to make a change to your current model, you will not need to fill out anything.
In addition to a direct email being sent to you with the form link, the "Request to Change Model" Form for Quarter 2 will also be posted to the district website beginning Monday, October 12 by 10:00am.
