Work Permit Information

To apply for a work permit:
 
  1. Fill out the "Work Permit Application" and have it signed by your parent or guardian.
  2. Ask your doctor to complete the "Physician's Certificate for Minor Work Permit" form. You can have a copy made of your school athletic or band examination to bring to us. It is good for one year.
  3. Ask your employer to complete the "Pledge of Employer" form.
  4. A birth cerlificate must be shown when applying for a work permit.  In cases when a birth certificate is unavailable, a hospital certificate or baptismal record will be accepted.
  5. After ALL of the needed sections have been completed, take them to Springboro High School so that you can receive the actual typed work permit to take to your employer. THE STUDENT MUST SIGN THE ACTUAL WORK PERMIT IN THE PRESENCE OF SPRINGBORO HIGH SCHOOL PERSONNEL.
